Application Process

Applicants applying for assistance or services will need to submit the following documents (where applicable):

  • Proof of tribal enrollment or descendancy verification;
  • Income verification for the past 6 months, including unemployment and/or disability payments;
  • Social Security income and/or Veterans benefits verification;
  • Birth certificates for all family members;
  • Residency verification;
  • Immunization records for child(ren);
  • Child care payment receipts or verification;
  • Child support paid or received;
  • Valid California Driver’s License or California ID card;
  • Utility bill;
  • Employment verification;
  • Social Security cards for all family members;
  • Aid verification;
  • MediCal/Food Stamp verification;
  • School attendance records;
  • Current checking/savings statement;
  • Car registration;
  • Proof of auto insurance;
  • Car payment verification;
  • Court-ordered custody documents;
  • Notarized Statement of Temporary Custody;
  • Selective Service verification.
